Please call the Trustee&#8217;s Office at 317-327-8947 to complete intake. You may expect a response time of 7-10 days. Thank you for your patience.<br \/>\n<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #ff0000; font-size: 11pt;\"><em><strong>NOTE: <\/strong><\/em><\/span><strong><span style=\"font-size: 18pt; color: #000000;\"><span style=\"font-size: 11pt;\">An appointment MUST be scheduled PRIOR to completing the township assistance application.<\/span><\/span><\/strong><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\">To process of your application for assistance, the below documents are required to accompany all applications:<\/span><\/p>\n<ul style=\"list-style-type: disc;\">\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Verification of income from ALL sources for the past 60 days (paycheck stubs, bank statements, pre-paid card statements, money order receipts, debit card printouts, etc.).<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Expense receipts and paid bill receipts from ALL payments made within the past 60 days.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Valid photo I.D. for ALL adults in the household.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Birth certificates for ALL children in the household.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Social Security cards for ALL members of the household or proof of application.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Lease agreement or mortgage statement or payment book if homeowner.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Divorce\/settlement papers (including Child Support printout).<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Work One printout.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Copy of income tax returns and W-2\u2019s.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Current utility bills, including phone, cable, auto, insurance, credit cards and prescriptions.<\/span><\/li>\n<li style=\"text-align: left;\"><span style=\"color: #000000;\">Additional information may be required by your caseworker.<\/span><\/li>\n<\/ul>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong>Other Public Assistance<\/strong><\/span><\/p>\n<p style=\"text-align: left;\">State law requires that all township assistance applicants\/recipients apply for other types of public assistance. Verification is required.<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong><span style=\"font-size: 14pt;\">Utilities<\/span><\/strong><\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\">Assistance can be provided for current utility bills as long as the bill is in the applicant\u2019s name and applicant\u2019s residence is current. Verification will be required.<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong><span style=\"font-size: 14pt;\">Food<\/span><\/strong><\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\">Food vouchers can be provided for households not receiving SNAP and meet eligibility requirements. Verification is required and some restrictions do apply.<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong><span style=\"font-size: 14pt;\">Transportation<\/span><\/strong><\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\">Bus passes can be provided for those eligible to maintain employment prior to receipt of first paycheck or to obtain necessary documentation needed for the intake appointment. Verification is required.<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong><span style=\"font-size: 14pt;\">Burials<\/span><\/strong><\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\">Assistance is available for deceased family members who lived in Warren Township and meet eligibility requirements. Verification is required and some restrictions apply. Contact the office for further information.<\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><strong><span style=\"font-size: 14pt;\">*School Uniforms*<\/span><\/strong><\/span><\/p>\n<p style=\"text-align: left;\"><span style=\"color: #000000;\"><span style=\"text-decoration: underline;\"><strong data-start=\"62\" data-end=\"192\">*We regret to inform you that, due to current budgetary constraints, school uniform vouchers will not be distributed this year.<\/strong><\/span><br data-start=\"192\" data-end=\"195\" \/>This decision was made after careful consideration of our limited resources.
